If you wish to contact your insurance company to ask what CPT Codes are covered under your policy, you may file with insurance. Check your out of network deductible. Find out the process to seek reimbursement with your company. I can provide a statement using those designated CPT codes for maximum benefit to attach to your insurance forms.
